• Your name and contact information, including an email address;
• A statement that you have a good faith belief the disputed use is not authorized by the copyright owner, its agent, or the law;
• A statement, made under penalty of perjury, that the information above is accurate and that you are the copyright owner or authorized to act on the owner's behalf;
• Your physical or electronic signature.

Counter-notification

It does not transmit or receive the counter-notice. The completed document must be delivered independently through a channel actually offered by the appropriate recipient and should include identification of the removed material and its former location, a statement under penalty of perjury that you have a good-faith belief the removal resulted from mistake or misidentification, your contact details, and any jurisdiction statement required by applicable law.

Repeat infringer policy

Sources or accounts associated with repeated, substantiated infringement notices are subject to restricted access to, or removal from, the Site at our discretion, in addition to removal of the specific material identified in each notice.
